• Go to the US Consulate or Embassy website.
• Download ‘Affidavit’ for certifying bachelorhood as below link http://photos.state.gov/libraries/india/482942/malikg/Marriage_001.pdf
• Fill the asked details in the form including name, date of birth, place of birth and the US correspondence and signature. It will be notarized by the notary officer.
• The ‘No Objection Letter’ will be issued only after screening. Check out the below sample of it.

• Submit it to US embassy online or in person.

• For confirmation, this form will appear online.
